Once a space has been confirmed for your child, the following forms and documents are required...
- Completed Enrollment Registration Form (click below)
- Completed Physical Exam Form (download below)
- TB skin test
- Copy of updated Immunization Records
- Copy of Birth Certificate
- Signed COVID 19 Waiver Form
- For Legal Guardianship, copy of court appointed legal guardianship document
- Copy of photo ID for both parents, legal guardians, all others authorized to pickup child
- Pais Registration Fee ($125 per child) required to secure enrollment. Fee is not refundable and not covered by Block Grant.
